All boys entering the 5th, 6th, 7th & 8th grades in the Fall (2008) and are members of either St. Joan of Arc Parish or St. Joseph's Parish in Maumee are eligible to participate.
Fall 2008 Program Information
- Sign-up period is from 5/1/08 to 5/31/08
- The participation fee is $65
- All equipment is provided expect football cleats (rubber only), and football pants.
- Practice will begin Monday, August 11
- Weigh-in will be Saturday, Sept. 6
- Round Robin will be Sunday, Sept. 7
- First Game will be Saturday, Sept. 13 or Sunday, Sept. 14
- All practices will be held at St. Joan of Arc
- All games are held at the CYO complex on Holland-Sylvania Rd.
- We try to have all 5th grade, all 6th grade, all 7th grade, and all 8th grade teams, depending on the number of boys in each grade.
- All rules are set by the CYO. Rules will be distributed to Players and Parents
- A mandatory player/parent pre-season meeting will be held on Sunday, August 10 @ 1pm in the SJA gym. All equipment will be distributed. Parent/Coaches meeting with practice times and schedule will be available.
What You Need to Do
- Fillout the registration form, player's contract, and emergency medical contact form. Each is linked below.
- Prepare a $65 check made payable to “St. Joan of Arc”. Please do not include other sports with this payment. This registration will be non-refundable after registration payment is made to CYO.
- Please remit the registration form with payment to the school office (marked "Football Registration") or mail to Jim Herrick, 4427 Talmadge Rd., Toledo, Ohio 43623.

Player's Contract
CYO requires a physician’s signature on the player’s CYO contract. This is an annual requirement. The form below should be completed by your family physician or physicals will be conducted at SJA in early August.
